Location & Parking
Located at 108 Medical Park Drive in Lexington, our office offers complimentary parking with ample spaces in the adjacent lot. We are easily accessible from I-85 and US-158.
New Patient Forms
Save time at your first visit by completing our online intake forms before you arrive. Forms will also be available in our office for those who prefer paper copies.
What to Bring
Please bring a valid photo ID, your insurance card, a list of current medications, and any relevant medical records or referral paperwork from your primary care provider.
